El estudio de la asociación entre el hipotiroidismo subclínico (HS) y la enfermedad cardiovascular (ECV) ha generado resultados contradictorios. No hay acuerdo acerca del nivel de TSH plasmática (TSHp) que definiría el HS, aunque recientemente se ha propuesto el valor ≥4.5 mUI/L. El objetivo ha sido estudiar la relación entre la TSHp y la ECV, analizando la TSH como una variable continua.
En 998 varones ≥50 años se analizaron las relaciones de la TSHp con 4 variables subrogadas de ECV, mediante modelos de regresión múltiple. La TSHp superior a la mediana (>1.36 mUI/L) se asoció significativamente con un riesgo incrementado de síndrome metabólico, hipovitaminosis-D y perfil lipídico aterogénico prevalentes. En ≥85 años, la TSHp fue un predictor de calcificación aórtica abdominal, tras ajustar por confusores.
En conclusión, en los varones≥50 años, valores de TSHp>1.36 mUI/L, que corresponderían a un estado eutiroideo, se han asociado significativamente con variables subrogadas de ECV.
The relationship between the subclinical hypothyroidism (SH) and the cardiovascular disease (CVD) has shown conflicting results. There is no consensus about the level of plasmatic TSH (pTSH) that should define the SH, though it has been recently proposed the value ≥4.5 mIU/L. The aim was to know the possible relationship between the pTSH and the CVD, analyzing pTSH as a continuous variable.
In 998 males ≥50 years, the associations between the pTSH and 4 surrogate variables of CVD were analyzed, using multiple regression models. Plasmatic TSH values greater than median (>1.36 mIU/L) were significantly associated with an increased risk of prevalent metabolic syndrome, D-hypovitaminosis and an atherogenic lipid profile. In subjects ≥85 years, pTSH was shown to be a strong predictor of abdominal aortic calcification, after adjusting for confounders.
In conclusion, in men ≥50 years, pTSH values >1.36 mIU/L, that correspond to an euthyroid status, have been significantly associated with surrogate variables of CVD.
